    PAYMENT
Do you accept insurance?
I do not accept insurance – I will provide Superbills with  Codes. It is up to you to get reimbursed. You can call you insurance company to find out what they cover. Each policy is different regarding deductibles, coverage parameters, length of coverage (number of visits allowed).

What forms of payment do you accept?

I accept Visa, Master Card, Personal Checks and Cash.
Do I receive a bill for services?
Payment is expected at each visit
What are your fees for services?
 I charge $150 for the first visit.
Subsequent visits are $100 unless they run more than an hour – 1.5 hour treatments cost $150.

LENGTH OF SESSION
What is the length of time for an appointment?
Most sessions run approximately one hour.
How many appointments will I typically have?
Most patients see me 4-6 times. Chronic conditions can take much longer.

 SCHEDULING

What are your hours for appointments?
 I work Monday – Saturday.
How long will I wait prior to my appointment?
I expect you to be on time – I will not keep you waiting.
What if I have to cancel an appointment?
Cancellations on the day of the appointment are charged $50 unless they are due to emergencies.

    HERBS AND SUPPLEMENTS

Will the herb and supplement cause side effects?
Herbs and supplements can cause side effects.
Are the herbs and supplements you provide safe?
All of our products come from the highest quality sources that test their products for purity, quality and accuracy.
Can I take the herbs and supplements you provide with my other medications?
 I make every effort to avoid side effects due to taking Herbs or Supplements while also taking medications.
Can Your Integrative Approach help me?
I have been able to help many people with health issues that do not respond to other approaches.

How can I best determine if your integrative approach will help my condition?
I do not want to waste your time and money if I cannot help you. The first meeting is the best way to determine what we can do.

    PREPARING FOR THE FIRST VISIT

What must I bring to the first visit?

Bring completed packet form filled out.
Bring a complete list of all medications and dosages.
Bring a complete list of current doctors, and other practitioners including their name and phone numbers.
Please bring an approximate description of previous surgeries, injuries, accidents to include what happened, and when.
Please bring some information regarding any allergies you have or suspect.
Please bring some information regarding any foods that cause discomfort of any kind whatsoever.
If you had a blood test in the last 90 days, ask your doctor for a complete copy and bring it, and any other lab tests in the last 90 days.
